Apex Beat and Its Clinical Relevance

The apex beat is a key clinical finding observed during a physical examination of the heart. It refers to the palpable cardiac impulse at the point where the heart's left ventricle makes contact with the chest wall.

Surface Marking of the Apex Beat

The apex beat is typically located at the fifth intercostal space in the midclavicular line. Variations in its position can indicate underlying cardiovascular conditions such as cardiomegaly or displacement due to lung pathology.

Clinical Significance

• Position: A laterally displaced apex beat may suggest left ventricular hypertrophy.
• Force & Duration: A forceful apex beat may indicate increased cardiac output, while a sustained impulse suggests hypertrophic cardiomyopathy.
• Absence: In conditions such as pericardial effusion, the apex beat may not be palpable.

Examination Method

To assess the apex beat:

1. Place the palmar aspect of your right hand over the left precordium.
2. Gradually refine palpation using the fingertips at the fifth intercostal space.
3. Observe its position, strength, and duration.
